The Program of All-Inclusive Care for the Elderly (PACE) Model of Care is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. West Health has embraced the PACE model as a gold standard of care for the frail elderly and is committed to its development and advancement.
Gary and Mary West PACE, a non-profit located in San Marcos, California, provides integrated, comprehensive medical and support services to seniors with chronic care needs while maintaining their independence for as long as possible. An interdisciplinary team is responsible for the assessment, development, implementation, and evaluation of the treatment plan of each PACE participant, with oral healthcare also provided through the co-located Gary and Mary West Senior Dental Center.
Gary and Mary West PACE can serve approximately 500 PACE participants, and opened in August, 2019.
Several research initiatives are underway including the impact of PACE on caregivers; identifying and addressing malnutrition and associated risk factors; palliative care strategies; acute care at home; reducing the use of potentially inappropriate medications, and implementing telehealth within the PACE care delivery model.
