Community-Based Palliative Care

We are working with leaders in palliative care to develop and evaluate healthcare delivery models that provide high-quality palliative care to patient and their families.

The Opportunity

Palliative care is specialized medical care for  people with serious illness – that improves  their quality of life by providing relief from the symptoms and stress of serious illness1.  It is delivered by an interdisciplinary team of physicians, nurses, social workers, chaplains, pharmacists and other practitioners to coordinate medical care and address the physical, intellectual, emotional, social, and spiritual needs of patients and their families. Individuals can receive palliative care at any stage in serious illness and it can be provided along with curative care.

With many seniors facing challenges associated with serious chronic illnesses, new models of palliative care that allow patients to receive their care in a variety of settings, have great potential to help those seniors maintain independence and improve their quality of life while preserving dignity.
