Our Work

Gary and Mary West Foundation

The Gary and Mary West Foundation partners with nonprofit community organizations to provide high-quality services to low-income older adults which help them remain independent and healthy for as long as possible. The Foundation is committed to supporting state-of-the-art senior centers that offer high quality nutritious meals and collaborate with other community based organizations to provide a wide array of services essential to the well-being of low-income older adults. In addition, we collaborate with our nonprofit partners and the West Health Institute to research and promote technologies that will improve the continuum of care and lower healthcare costs for seniors in our community.

We’re researching solutions for seniors and their caregivers to better enable successful aging.

  • Geriatric Emergency Care

    The emergency department has become an increasingly common gateway to medical care for Americans, but given the unique needs of older adults, there is a great opportunity to improve care pathways for seniors in emergency settings.

  • Home-Based Primary Care

    We are researching more effective ways to care for seniors at their homes, putting their needs first and improving access to high-quality, affordable healthcare.

  • Emergency Department-to-Home

    Emergency Departments are the source of almost half of all admissions to hospitals. What if there were more senior-centered, home-based alternatives to transition seniors to from the ED?

  • Long-Term Services and Supports

    We are researching how helping provide care coordination, meals, and support at home can help seniors age more successfully and independently in place.

  • Palliative Care

    We are researching how new models of high-quality palliative care can become widely adopted wherever it’s best for the patient and their families.

  • Caregivers

    We are researching a comprehensive, cross-disciplinary care delivery approach to meet the needs of both caregivers and the recipients of care, ensuring the foundations of Successful Aging are more fully realized.
